Why full-arch implants need professional maintenance

Full-arch implant restorations are built to last, but the area beneath the prosthesis cannot be fully cleaned at home. Over time, plaque, bacteria, and hardened deposits can build up where you cannot reach, and that buildup is what puts the health of the implants and gums at risk.

Professional maintenance every six months lets our ISFP team:

  • Remove the prosthesis and thoroughly clean the areas underneath that home care cannot reach.
  • Clear away plaque, bacteria, and calculus that can contribute to peri-implant disease.
  • Check the health of the implants, the surrounding tissue, and the supporting components.
  • Inspect the screws, the fit of the prosthesis, and how everything functions together.
  • Catch small issues early, before they turn into bigger repairs.
  • Help reduce the risk of implant complications and support the long-term life of your restoration.

Routine care now is what helps you avoid costly problems later.

Signs it may be time for a maintenance visit

Some patients come to us because something has started to feel different. Others come simply because it has been a while. Both are good reasons.

It is worth booking a visit if you notice:

  • Food packing underneath or around the prosthesis more than it used to.
  • An odor or taste that does not clear with brushing and rinsing.
  • Gums that look red or puffy around the implants, or that bleed when you clean.
  • Tenderness or soreness in an area that was previously comfortable.
  • A prosthesis that feels different when you bite, or that seems to have shifted.
  • Finding it harder to keep the area clean than it once was.

One important note: in the earlier stages, problems around implants are often painless and easy to miss. The absence of symptoms is not confirmation that everything is healthy underneath. That is precisely why maintenance is scheduled rather than left until something feels wrong.

If it has simply been more than six months since your prosthesis was professionally removed and cleaned, that on its own is reason enough to book.

Understanding peri-implant disease

Implants themselves cannot develop cavities, which leads some patients to assume they no longer need regular care. The tissue and bone that support those implants can still be affected, and that is what maintenance is designed to protect.

Peri-implant problems generally fall into two stages:

  • Peri-implant mucositis is inflammation of the soft tissue around an implant. The gums may look red, feel tender, or bleed during cleaning. At this stage the condition is generally considered reversible with professional cleaning and improved home care.
  • Peri-implantitis is the more advanced stage, where inflammation extends to the bone supporting the implant and bone loss can occur. This is considerably harder to treat, and in some cases it can threaten the stability of the implant itself.

The reason the six-month schedule matters is that the earlier stage is far easier to manage than the later one. Regular professional cleaning and assessment gives our team the opportunity to identify changes while they are still straightforward to address.

  • Prosthesis removal, professional cleaning, and reinsertion — your restoration is removed, cleaned, and secured back in place.
  • Implant maintenance exam — an evaluation of your implants, gums, and the prosthesis.
  • Radiographs when clinically indicated — imaging to check what is happening below the surface, included as part of your maintenance exam.

What to expect at your visit

If you have never had a full-arch prosthesis professionally removed before, it helps to know what the appointment involves.

  1. Review and examination. We start by asking how you have been getting on, anything you have noticed, and how your cleaning routine is working day to day. Your gums, implants, and prosthesis are then examined.
  2. Prosthesis removal. The retaining screws are accessed and the prosthesis is carefully lifted away. This is a routine, planned procedure and is generally comfortable for patients.
  3. Cleaning underneath. With the prosthesis out, the implants, abutments, and surrounding tissue can be cleaned properly, including the areas that no toothbrush or water flosser can reach.
  4. Assessment of the tissue and components. We check the health of the gums around each implant and inspect the screws and fittings for wear or looseness.
  1. Imaging where indicated. If radiographs are appropriate, they are taken and reviewed with you as part of your exam.
  2. Cleaning and reinsertion of the prosthesis. The restoration itself is cleaned thoroughly, then repositioned and secured.
  3. Your plan going forward. We talk through anything we found, adjust your home care routine if it would help, and schedule your next visit.

Most patients are in and out in a single appointment and return to normal eating and speaking the same day.

Caring for your implants between visits

Good home care will not replace professional maintenance, because no home tool can reach beneath a fixed prosthesis. It does, however, make a real difference to the health of the tissue around your implants.

What we generally recommend:

  • A water flosser, directed along the gumline and underneath the bridge, used daily.
  • Interdental or specialty brushes sized to fit the spaces around your implants.
  • Floss threaders or implant-specific floss where your team has shown you they will fit.
  • A soft brush and a non-abrasive toothpaste, avoiding heavily abrasive or whitening pastes that can dull the surface of the restoration.
  • Keeping an eye on the gumline and mentioning any bleeding, swelling, or change in fit at your next visit.

Restrictions apply, and all patients require a clinical evaluation to determine eligibility for treatment. Additional charges may apply based on individual clinical needs. We accept PPO insurance plans and offer financing options.

You do not have to return to the practice that placed your implants for ongoing maintenance.
